Make an Appointment

Services of the Hearing and Speech Clinic are available to anyone who seeks them, regardless of age. While referrals are frequently made by medical, dental, and educational personnel for evaluation and/or treatment services, it is necessary for the client or parent/guardian to make the initial contact requesting services. The University of Maryland Hearing and Speech Clinic is a training and community service facility, therefore services are provided for reasonable rates.

Prior to your appointment, please fill out the applicable case history form. 

NOTE to Potential Clients Seeking Speech-Language Services: Following an appointment request, a standardized form requesting information is mailed or can be downloaded using the appropriate link above. The client or parent/guardian must return the completed form and should have any relevant medical, psychological or educational forms forwarded to the clinic. An evaluation appointment will be scheduled after the requested information has been returned.
